What are Community Care services?
Your loved ones may require medical help or care as they recover at home. They also need support with daily life, such as to eat and get dressed. There are community-based services available to support them and you, as their caregivers.
Meanwhile, individuals who are in good health may maintain their health via social activities.

Residential (Stay-In) Care
This refers to long-term care being provided to seniors, which includes:
Inpatient Hospice Palliative Care Service
Inpatient Hospice Palliative Care Service serves end-of-life patients who cannot be cared for at home and require inpatient care.
Nursing Home
Nursing Homes are long-term residential care facilities that provide assistance to residents who need help in most of their activities of daily living and/or have daily nursing care needs.
Home Care
This refers to home and personal services support your loved one in his/her private space, familiar surroundings and comfortable environment or at home. This can include:
Meals on Wheels
The service provides meal deliveries to homebound clients who are unable to buy and prepare their own meals and do not have a caregiver to help them to do so.
Home Medical
For those who are frail and homebound, and require help with medical needs at home.
Home Therapy
Home Therapy provides home-based rehabilitation services such as physiotherapy, occupational therapy and speech therapy that support clients in recovering their functional abilities.
Hospice Home Care
Patients with life-limiting illnesses, such as advanced dementia or cancer, and whose conditions are expected to get worse in weeks or months may choose to spend their days at home instead. In such cases, caregivers need help coping
Medical Escort and Transport
The service provides transportation and/or someone to accompany clients to travel to and from their homes for medical appointments and treatments (for example, at hospitals, specialist outpatient clinics, or polyclinics).
Home Nursing
For homebound clients and their caregivers who need assistance on nursing care at home.
Home Personal Care
For those who require personal services to continue living well at home or to help support their caregivers in caregiving duties.
Befriending Service
Volunteers from befriending services can help to take care of the emotional and psychological wellbeing of these old folks by helping them or listening to their problems.
Centre-based Care
This refers to care services which provides day programmes for seniors who are suitable for centre-based care setting and require supervision while caregivers are at work. Such service helps to maintain and/or improve the general, physical and social overall well-being of loved ones through therapy. Some centres double up as a place for loved ones to socialise and take part in recreational activities. This can include:

Day Care provides a full day programme for seniors who are suitable for centre-based care setting, and require supervision while their caregivers are at work.
Community Rehabilitation Centre
Community Rehabilitation Centre provide speech or physiotherapy for seniors who need to regain their abilities.
Senior Activity Centre
These centres conduct activities such as karaoke, arts & craft and exercise sessions to keep the elderly engaged.
Centre-based Nursing
Centre Based Nursing provides basic nursing care for seniors who need wound management help or assistance with nasogastric tubes and other equipment.
