- Duplication of efforts as handover notes are a summarised version of notes keyed into system
- Deciphering notes may be difficult and time-consuming if handwriting is illegible
- Documentation fields are in separate forms, which require staff to navigate or open multiple tabs to complete documentation for one senior

Solution
Using templates for documentation, digitising handover notes, and using mobile-enabled applications for data entry can reduce inefficiencies in previous processes.
The desired outcomes include:
- Reduced overall time taken for documentation
