Supporting AACs’ IT Capabilities

Previously known as the Eldercare IT Grant back in January 2022, the grant has now been revamped as the AAC IT Grant to meet the service needs and tracking/reporting of relevant indicators under the new Active Ageing Centre (AAC) service model.

This grant aims to support AACs in their digitalisation needs – these could include procuring new IT capabilities or making enhancements to current IT capabilities.

Background

Active Ageing Centres (AACs) form a population-based service model and they serve as the go-to point for all seniors in their community.

AACs provide the “ABC+2S” suite of services: Active Ageing, Befriending, Buddying, Information and referral to Care services, Social Connector for social and lifestyle interventions and Community Screening.

AAC (Care) are centres that provide care services, such as day care and community rehabilitation, in addition to the services under the AAC service scope.

In support of the AAC service model rollout, it is essential for AACs to adopt technology as the cornerstone to design data-informed services for greater efficiency and as a single source-of-truth.

The current shortlisted AAC IT systems and vendors will be superceded by whitelisted AAC IT systems and vendors in late 2024.

These whitelisted systems will meet requirements of the AAC2.0 service model and beyond.

Grant quantum

With effect from 1 March 2024, the supportable grant quantum is up to $83,000.

Applications for the AAC IT Grant are open on a quarterly basis for a duration of one (1) month:
