Please note that the form below is only for CCOs to submit a cumulative list of all AH incidents towards healthcare workers that occurred in the reporting calendar year (1 January – 31 December) to AIC/MOH.

Description	Please use this form only for submission of all AH incidents towards Healthcare Workers in the reporting calendar year (1 January – 31 December). Please note that all FormSG submissions should be sent in by 7 January. For submission, please ensure that: All required fields are completed in the annual batch reporting template. The necessary approvals from your organisation’s CEO (or equivalent) and other appropriate approving authorities (if any) have been sought before submission to AIC/MOH. The excel file is password-protected before being uploaded onto FormSG for submission, and password to be sent to AH Incidents (AIC).
