Background
The Community Care Digital Transformation Plan (CCDTP) is an initiative that seeks to uplift and accelerate digitalisation efforts in the Community Care sector. It aims to provide better care and support for clients and their caregivers in the community, increase productivity and job satisfaction by leveraging technology within Community Care organisations (CCOs), and deliver timely, affordable and quality care for Singaporeans via a digitally driven sector.
Basic IT Funding for Nursing Homes
Background
The funding is developed for the nursing home sector to improve productivity, interoperability, and clinical outcome of clients through digitalization. Concurrently, also to provide support to nursing homes to enable compliance with select upcoming bills such as the Health Information Bill, with regard to data contribution to the National Health Electronic Records (NEHR).
Funding support and eligibility
Identified Nursing Homes licensed by Ministry of Health who has yet to adopt a Clinical Management System, and who must fulfil a set of KPIs accordingly.

Productivity and Digitalisation Grant
Background
The CCDTP is developed by the Community Care Technology Council*, supported by the Agency for Integrated Care and Ministry of Health, in partnership with other stakeholders such as National Council of Social Service.
The CCDTP provides a structured and targeted approach to support CCOs in their digitalisation journey, focusing on the following strategies:
- Digitalisation journey map
- Business process re-engineering
- Technology adoption
- Data-driven and capability uplift
A three-level Sector Digitalisation Roadmap will guide CCOs’ decision making in adopting proven technology, building digital capabilities and leveraging resources to enhance their organisational effectiveness and delivery of services.

* The Community Care Technology Council comprises MOH, AIC, AWWA Ltd, AMKFSC Community Services Ltd, NTUC Health, Ren Ci Hospital, SATA CommHealth, Sree Narayana Mission, St Andrew’s Community Hospital, St Andrew’s Nursing Home, St Luke’s ElderCare Ltd and TOUCH Community Services Ltd, Synapxe and Infocomm Media Development Authority. This is for the period of 1 July 2021 – 30 June 2023.
Funding Support
CCOs may apply for Productivity and Digitalisation Grant (PDG) to defray the costs of adopting technological solutions, as part of their digitalisation journey.
NEW! Under the revised PDG funding scope following scheme extension, only Green Lane Projects* will be considered and processed on a first-come first-serve basis:
- Up to 85% of approved project costs comprising of CAPEX and 1 st year OPEX
- Funding capped between $30,000 – $150,000 per project

Eligibility
- A legal entity registered under Accounting and Corporate Regulatory Authority (ACRA) / Registry of Societies (ROS) / Registry of Co-Operative Societies (RCS) or gazetted in AGC
Statutes. - A Singapore-based organisation that provides community care services in line with MOH’s licensing or service requirements.
- Project should not be used for replacement of existing solution or equipment.
- Project should not have received or will be receiving other sources of government funding for the portion that PDG will be funding.
- Project should not have commenced or confirmed the commencement.
Grant Application Process and Basis of Project Evaluation
With note of indicated timeline (see below), interested applicants may:
1. Approach your Account Manager or or PDG Team at ccdtp@aic.sg.
2. Draft your application(s) early using this this supplementary form (one form per project, per site), and seek the PDG Team’s inputs and advice.
[Note: Applicants must ensure that all intended projects shall be completed by 15 October 2025. Do reach out for assistance should you foresee that the stipulated deadline cannot be met.]
3. Submit your completed project proposal(s) on OurSG Grants (OSG) Portal by 1st February 2025, only when advised by the PDG Team.
Projects will be evaluated by a panel, based on feasibility, scalability, sustainability, productivity and other outcomes – minimum KPIs to fulfil:

- Productivity gain by at least 10% (e.g. man-hours saved / cost savings / clients served);
- Completion of project with go-live; and
- Sustained usage of solution for a minimum of three (3) years from go-live.

4. Expect to receive application outcomes by 31 st March 2025 (end of PDG scheme).
