Support In The Community
The Agency for Integrated Care (AIC), together with the Ministry of Health (MOH), developed the Community Mental Health Masterplan in 2012 which enables persons with mental health and dementia to seek early treatment nearer to their homes and ensure that they are well supported in the community. The Masterplan focuses on access to care and promote early identification of mental health conditions, integrating health and social care for those with such conditions, and building a dementia care network.
To do this, AIC works to build an integrated community mental health network across various care settings and sectors to support persons with mental health issues and their caregivers, so that they can age well in the community. Our partners in this network include General Practitioners (GPs), Polyclinics, as well as Social Service Agencies (SSAs) and community partners who run the community outreach teams, community intervention teams, specialist-led teams and other care and support services.

Programme Details
CREST-CSN focuses on caregivers who are caring for persons with mental health issues and/ or dementia. These individuals may have or are at risk of developing depression, anxiety and burn-out themselves as a result of their caregiving responsibilities. We increase public awareness of caregiving concerns and promote early identification of caregiver needs.

CREST-Youth focuses on raising awareness of mental health conditions and promote early identification of mental health needs in youth. We provide emotional support to youth in psychological distress and actively involve the caregivers of youth in their recovery journey. We also serve as the community node linking youth and their families to appropriate health and social support networks when necessary.
COMIT provides psychotherapy and psychosocial intervention such as cognitive behavioural therapy and psychoeducation support for clients with mental health issues and dementia, and their caregivers. We work closely with the GPs, polyclinics, hospitals and other community partners to provide collaborative and coordinated care to clients and caregivers in the community.
CREST-Youth focuses on raising awareness of mental health conditions and promote early identification of mental health needs in youth. We provide emotional support to youth in psychological distress and actively involve the caregivers of youth in their recovery journey. We also serve as the community node linking youth and their families to appropriate health and social support networks when necessary.
The Aftercare programme provides transitional care support for those who have mental health issues with complex medical and social needs, for eg non-adherence to treatment and appointment, frequent relapse episodes etc. We facilitate their discharge, sustain their psychological well-being and re-integrate them back into the community.
ASCAT supports the polyclinics, GPs and community mental health partners with clinical consultation and capability building, while providing gatekeeping to hospital services. All cases with more urgent and severe mental health needs surfaced by Mental Health General Practitioner Partners and Community Intervention Team (COMIT) will be triaged by the ASCAT psychiatrist to determine if the fast track service is required at the hospital.
Those with mild to moderate mental health and chronic health conditions can seek holistic treatment at a nearby GPs or polyclinics, in a less stigmatising environment.
CREST-PDS provides education on the condition, coordinates community resources and facilitates early planning and management of the condition, for newly-diagnosed persons living with dementia and their caregivers for six months from the time of referral.
