Introduction
Day Hospice Care supports patients with serious illness, like advanced dementia or cancer, with palliative care and social activities at a centre. It also supports their caregivers who may find it hard to manage their loved one’s changing needs during this period.
Who is this service for
This is for clients who:
- Have been diagnosed by a doctor to have a serious illness with a prognosis less than a year
- Do not require 24-hour nursing assistance
Services provided
Services may include:
- Palliative care: Medical and nursing support to manage pain and symptoms
- Psychosocial support: Emotional and social support for both patients and caregivers during this difficult period
- Transportation for patients to and from their homes
- Games and activities to keep patients engaged

Cost
Please check with your service providers on the cost. The amount varies based on your household’s income.
To qualify for subsidies, your loved one will need to be a Singapore Citizen or PR.
Subsidies are also means-tested (i.e. there is an income criteria).

How to apply
Do talk to a doctor (GP or a doctor from your hospital or polyclinic) who is familiar with your loved ones’ condition and needs. They will help to make the necessary referral.
